Since 1984, the California Alzheimer's Disease Centers have been centers of excellence promoting model clinical care for a complex diagnosis though diagnostic services, education, and providing support to families by:
- providing diagnostic and treatment services
- improving the quality of care for persons with Alzheimer's disease
- providing education and support to the families of persons with Alzheimer's disease
- increasing and improving the training of health care professionals with respect to Alzheimer's disease
Who is eligible to receive an assessment at the CADCs?
Any individual with symptoms of memory loss, disorientation, and confusion may contact the Centers for an evaluation. Patients may be self-referred or referred by family, private physician or community agency.
Are the assessment services covered by insurance?
Insurance arrangements and fees vary from one CADC to the other. Refer to your CADC to find out specific cost information. Each of the CADCs tries to make it possible for patients to be seen regardless of their economic circumstances.
The following assessment and intervention services are usually available at CADCs:
- review of medical history, medications, and psychosocial history
- psychosocial assessment
- multidisciplinary team meeting
- family conference
- care plan
- written summary of the diagnosis, assessment, and care plan for the patient, family, and referring physician
- discussion of issues such as driving, durable power of attorney for health care, legal and financial counseling, elder abuse, home safety measures, and safe return registry
- patient/caregiver support groups
- referral to community services
- referral to research studies/clinical drug trials
- phone follow-up visits
